Workshop Registration Form

Please carefully read our workshop registration process before submitting the form and making the payment. To qualify as low-income, an individual must receive one of the following benefits, SNAP or TANF Benefits. Only SNAP or TANF benefits qualifies an individual as low-income for this program (Proof of low-income status will be required). To participate as a low-income individual, please submit payment through Zelle in the amount of $200.00 (cost for low-income individuals). Please note that each individual who wishes to participate must pay the fee of $200.00 if they qualify as low-income. The participation fee of $200.00 for a low-income individual does not include bringing a spouse, friend, relative or co-worker etc.. With the exception of small children under the age of 10 years, each attendee must pay the required participation fee.

We do not offer sponsorships, or any other type of assistance for payment to participate at our workshops. What if you are not low-income but would like to participate? Answer:

Please note that each individual who wishes to participate must pay the $499.00 fee. The participation fee of $499.00 does not include bringing a spouse, friend, relative or co-worker etc.. With the exception of small children under the age of 10 years, each attendee must pay the required participation fee. Please note, the fee to participate virtually is the same fee as in-person participation fees.

Important Registration Policy

Because workshop seating is limited, registration is confirmed only after both a completed registration form and full payment have been received. Submitting a registration form alone does not reserve a seat.

Seats are assigned on a first-paid, first-confirmed basis. If payment is not received promptly, your registration will remain pending, and your seat may be offered to another participant without notice.

To ensure fairness to all participants and to prevent reserved seats from remaining unpaid, individuals who repeatedly submit registrations without completing payment may have future workshop registrations declined at the discretion of DFOWW Incorporated.

Group discounts are available for students and senior citizens. For additional information, please contact our main office at (678) 977-9703.

Participants will be responsible for printing and bringing all the documents including applications and forms we will discuss and review during the workshop. Media Release & Photography Policy. DFOWW Incorporated photographs and records videos during its nutrition workshops, food distributions, volunteer activities, fundraising events, community outreach initiatives, educational programs, meetings, and other charitable activities to document and share the impact of our mission.

By attending, participating in, or volunteering at any DFOWW Incorporated program, workshop, food distribution, fundraiser, meeting, event, or activity, individuals acknowledge and consent to the photographing, filming, and recording of their image, voice, and likeness by DFOWW Incorporated or its authorized representatives.

Participants grant DFOWW Incorporated the unrestricted, perpetual, worldwide, royalty-free right to use, reproduce, edit, crop, publish, distribute, display, and otherwise use such photographs, videos, and recordings, without compensation, for charitable, educational, promotional, fundraising, marketing, grant reporting, newsletters, annual reports, social media, the DFOWW Incorporated website, and other mission-related purposes.

All photographs, videos, and recordings created by or for DFOWW Incorporated remain the property of DFOWW Incorporated.

For participants under 18 years of age, a parent or legal guardian, by registering the child or permitting the child to participate, acknowledges and agrees to this Media Release & Photography Policy on the child’s behalf.

Participation in any DFOWW Incorporated program, workshop, food distribution, volunteer activity, fundraising event, meeting, or other activity is conditioned upon acceptance of this Media Release & Photography Policy. Individuals who do not consent to being photographed, filmed, or recorded are not eligible to participate.

DFOWW Incorporated reserves the right to use photographs, videos, and recordings taken during its activities for any lawful charitable, educational, promotional, fundraising, or organizational purpose consistent with its mission.
