Workshop Registration Form

Please carefully read our workshop registration process before submitting the form and making the payment. To qualify as low-income, an individual must receive one of the following benefits, SNAP or TANF Benefits. Only SNAP or TANF benefits qualifies an individual as low-income for this program (Proof of low-income status will be required). To participate as a low-income individual, please submit payment through Zelle in the amount of $200.00 (cost for low-income individuals). Please note that each individual who wishes to participate must pay the fee of $200.00 if they qualify as low-income. The participation fee of $200.00 for a low-income individual does not include bringing a spouse, friend, relative or co-worker etc.. With the exception of small children under the age of 10 years, each attendee must pay the required participation fee.

We do not offer sponsorships, or any other type of assistance for payment to participate at our workshops. What if you are not low-income but would like to participate? Answer:

As a courtesy, individuals who do not meet the definition of low-income are permitted to participate by paying a $499.00 fee. Please note that each individual who wishes to participate must pay the $499.00 fee. The participation fee of $499.00 does not include bringing a spouse, friend, relative or co-worker etc.. With the exception of small children under the age of 10 years, each attendee must pay the required participation fee. Please note, the fee to participate virtually is the same fee as in-person participation fees.

Important Registration Policy

Because workshop seating is limited, registration is confirmed only after both a completed registration form and full payment have been received. Submitting a registration form alone does not reserve a seat.

Seats are assigned on a first-paid, first-confirmed basis. If payment is not received promptly, your registration will remain pending, and your seat may be offered to another participant without notice.

To ensure fairness to all participants and to prevent reserved seats from remaining unpaid, individuals who repeatedly submit registrations without completing payment may have future workshop registrations declined at the discretion of DFOWW Incorporated.

Participants will be responsible for printing and bringing all the documents including applications and forms we will discuss and review during the workshop.
